What to Have Ready
Operative notes, discharge summaries, histology and laboratory results, imaging files, and clinical photographs, together with the proposed plan and the questions you want answered.
Send copies and keep your originals. If a document cannot be obtained, say so — an acknowledged gap is workable, an unacknowledged one is not.
